Retrospective drug utilization review of cisapride in pediatric patients with gastro-oesophageal reflux and gastric dysmotility |

Summary : Objective: Describe and evaluate cisapride utilization in pediatric patients suffering from gastro-oesophageal reflux and gastric dysmotility. Methods: Retrospective drug utilization review of patients with cisapride prescription in CHU Sainte-Justine. Demographic characteristics and diagnosis tests were collected form the medical charts. Information related to others pharmacological drugs prescribed in treatment of gastro-oesophageal reflux was collected from the computerized pharmaceutical patient file. Finally, the evaluation of cardiac follow-up was conducted by searching for electrocardiogram performed before and after initiation of cisapride therapy in the medical charts. Results: The last fifty patients who were prescribed cisapride before July 17 2008 were selected. The median age was 7 months (<\; 1 month-16 years old) and 67% were premature. Most frequently identified indications in our cohort were gastro-oesophageal reflux (88%) and respiratory disease such as pneumonia and asthma (62%). Main treatment identified in first-line, second-line and third-line of treatment were respectively histamine receptor antagonists (ranitidine) (60%), proton pump inhibitors (40%) and prokinetic agents (cisapride) (49%). An electrocardiogram was performed in 92% and 87% of premature patient aged between 0 and 3 months respectively before and after initiation of cisapride prescription. Conclusion: Despite withdrawal from the market in July 2000, cisapride remains an important drug used when conventional therapies have failed for the treatment of gastro-oesophageal reflux. Cisapride also remains the only efficient prokinetic agent for pediatric patients younger than 3 years old suffering from gastro-oesophageal reflux. |
